Reserve Your Appointment
Follow the instructions below to complete the reservation process.
Step #1: Complete the Pre-Screening Form. This is required every time you book an appointment for DEXA & VO2Max services.
Step #2: Book your appointment using the drop down menu on the page for the service(s) you want. If you are booking more than one service you will have to book them separately if they are not already listed together.
Be sure you have read and understand the Booking Rules before proceeding.
BOOKING POLICY REMINDER
You will be required to enter your credit card information upon booking your appointment regardless of membership status or voucher purchase.
You will not be charged unless you violate our booking policy. You can read our booking policy here.
Once you arrive for your appointment, you will be asked if you'd like to change your payment method and may do so at that time.

To book an appointment, you must complete this medical clearance questionnaire EVERY TIME you reserve.
Please only submit 1 FORM per service date even if you are having more than one service on the same day.
(Please note that completing this survey DOES NOT reserve an appointment for you. You must complete Step #2.)

IMPORTANT: Use in CHROME or SAFARI browsers only. Make sure your pop-ups are ENABLED.
RMR Analysis and VO2Max require 6 hours of fasting prior to testing. Please keep this in mind when scheduling.
YOU ARE NOT PERMITTED to book appointments for another person. Each person must have their OWN account. Any duplicate services booked under the same account on the same day will be immediately cancelled.
How To Reserve:
1.) Select a Service from the drop-down menu.
2.) Select a date on the calendar and hit SEARCH. It will bring up 7-days at a time.
3.) Choose a date and time that works for you.
If you reload this page multiple times, you WILL receive an error.
BEFORE contacting us, make sure you:
1. DELETE your browser history & EXIT your browser, then RELOAD the browser.
2. Make sure you are using a SMART PHONE or COMPUTER and NOT an IPAD.
3. Go through THIS STEP of the booking process again ONLY. DO NOT repeat Step #1.
You MUST enable EMAIL notifications when you create a MindBody Account or you will not receive appointment confirmations or pre-test prep instructions.
